Community Health Profiles (CHPs) help describe community health status. CHPs can assist communities with making a positive impact on their health system by presenting key health measures to describe, measure, and track the health status of the community. USET, HPDP, and DEDP are collaborating on designing a scalable CHP production methodology designed to accommodate a continuum of:

- category and measurement selection

- data access

- population aggregation

- data display and communication

- production sophistication and capacity
