Patient-Centered Medical Home (PCMH) Model Virtual Training

The USET Office of Tribal Public Health (OTPH) is offering a Patient-Centered Medical Home (PCMH) virtual training to USET member Tribal Nations and other IHS Nashville Area Tribal Nations, USET staff, and NAO IHS staff. The PCMH model is an approach to delivering high quality, cost-effective primary care. Using a patient-centered, culturally-appropriate, and team-based approach, the PCMH model coordinates patient care across the health system. The PCMH model has been associated with effective chronic disease management, increased patient and provider satisfaction, cost savings, improved quality of care, and increased preventive care.
When: May 27 at 1:00 pm – 3:00 pm Central/2:00 pm – 4:00 pm Eastern
Who should attend: Tribal Nation clinic staff, IHS clinic support staff, and USET OTPH staff.
Learning objectives: Participants will gain an understanding of:
- The function of a PCMH.
- How to create a more efficient and culturally-centered approach to whole-person health care delivery through the core principles of PCMH.
- The benefits of PCMH.
